Making Referrals to Olympia Athletics

Our physiotherapy and exercise fitness training sessions are open to general public and caters for anyone from the elderly wanting to improve their daily function, to elite athletes returning to sport.

To make a referral, please either provide your patient with a physical referral or email your referral letter to info@olympia-athletics.com.

Fees

We are proud to offer physiotherapy services at highly affordable rates allowing us to cater for the masses. At a rate of $65 per 20 minute session, our typical gap payment is $4.65 per session with a Medicare referral.

We also offer flexible appointments after hours and on Saturdays.

Chronic Disease Management (CDM)

Formerly Known as Enhanced Primary Care (EPC)

CDM was introduced to help patients reduce the cost of allied health services through the Medicare system to improve patient care and treatment outcomes.

GPs can prescribe physiotherapy referral to patients who has:

  1. A medical chronic condition;

  2. One or more medical conditions that have been present for at least 6 months

  3. Complex care requirements (considered for patients who require ongoing care from multidisciplinary team) managed by a medical practitioner (other than a specialist or consultant physician) under both a GP Management Plan and Team Care Arrangements or, if the patient is a resident of an aged care facility, the patient’s medical practitioner has contributed to a multidisciplinary care plan

Patients are eligible for up to a maximum of 5 physiotherapy sessions in a calendar year.

There is no set list of eligible conditions. If the condition is chronic (i.e. has been or is likely to be present for six months or longer), it can be treated through CDM.

Conditions treated under CDM

Please note that Medicare requires a valid referral from a GP for physiotherapists to conduct sessions through CDM.

The number of services stated in the referral is a ‘course of treatment’. The maximum number of sessions you can include on a referral for the initial course of treatment is five.

After each course of treatment, the treating physiotherapist will provide you with a progress letter and may request additional sessions. A new referral is required for each course of treatment.
